As one who has seen plenty of cases where PCs are addressed with the pronoun appropriate to the player rather than the PC - will it really be a good thing if every time this is done it sparks off a plot thread and possibly ruins the disguise (and possibly the character concept) of the cross-dressing PC?
I can easily foresee: "well, our friend over there, he.." "He!?" "Uhhmmm..." [20 mins fixing the gaffe] "Phew, that was close. If only he hadn't.. oops."
Mind you, some players seem to *expect* to be addressed with their own pronoun rather than their PCs. When I was in a D&D group I wasn't familiar with I got a VERY shocked reaction when I addressed a male player with a female PC as "Good lady..".
Oh, and using illusions to appear as a noblewoman was a VERY popular was of smuggling weapons into secure installations in one game. :)
