You missed the most obvious of beginnings of them all:
"You all meet in a tavern (or pub, or bar...)" and then go on from there! ; )
In all seriousness, you are correct, that the humble or not so
humble meeting of players is forgotten or ignored.
Another suggestion:
Tell the players where they are going to meet up: then let them
tell a story about who they are and how they got there, and when
all the stories have been told, only then does the GM step in and
start the adventure, tailoring the whole meeting up incident to the
character's own individual stories.
In other words, let the players set the initial tone to the campaign,
and let the world (i.e. the GM) take over... So that the campaign
does not begin with the meeting, but with how the characters' individual
paths led to this point.
So for me, I would have named the column:
"So the adventure continues..."
Just some thoughts...
