I had one player who always cancelled at the last minute, and when he turned up always sat in the corner playing handheld computer games or sketching.
Was nervous about kicking him from the group, and tried the softly softly lets have a chat technique. Nothing changed so I booted him, and since then the game sessions have been an order of magnitude more fun for everyone involved.
So my addendum to teh above advice is: GM's/hosts don't be afraid to ditch players who can't observe the above courtesies
