I personally prefer game designer as it bespeaks, at least at first glance, a greater effort. Not only do you have to write the thing, but you have to understand all sorts of other design issues. The designer is truly a jack of all trades.
That said, it might make a better impression. Of course, hopefully, fairly shortly I'll be able to add novelist and game designer (err, roleplaying game author) to my resume, and novelist and RPG author might be a bit redundant in its effect even if the actuality is fairly widely separated.
