This article is quite right, communication is definately a problem in many games.
As far as miniatures go, I hate them. But I have found a way to minimize confusion in some situations, at least where character positioning is concerned.
In the army we had procedures covering many situations. For example, in MOUT (military operations in urban terrain) training we learned procedures for clearing buildings. These were important because you were less likely to shoot or blow up other guys in your squad.
I have had parties do the same. After all, an adventure party is supposedly a group of professionals who face life-threatening situations all the time, just like soldiers. I am sure they would eventually develop some procedures instead of just stumbling into every encounter. Of couse this can't always help, but having procedures in place can help in some situations, and the GM can always use them as a default when the players don't fill in the details as to what they are doing.
