I think it helps to look at a campaign not as either predetermined or protagonist, but to see to what extent they have the elements of wach type.

I run games mostly in the protagonist sense described here, but there will be events that are scripted, that will occur regardless of player actions. Similarly, I make notes before a game about ways to encourage characters to act within certain limits, to keep a game on track, mostly game mechanisms to keep the game together. But I don't do this to that great an extent, and by and large players are left to drive things by themselves.

I think it comes to a spectrum of games between the two situations given, with predetermined at one end, and protagonist at the other (with 'winging it' perhaps beyond that?).
